We value providing quick and convenient access to needed services. Individuals can refer themselves, be referred by family members, or be referred by outside service providers such as hospitals and social service agencies. The first step for any type of referral is to call us between the hours of 9 am and 5 pm Monday through Friday and ask to speak to our Intake Coordinator (973) 773-7600 or to complete an online referral/contact form.

 

For individuals and their family members, please complete the SELF-REFERRAL FORM.

 

For service providers, please complete the SERVICE PROVIDER ONLINE REFERRAL FORM.

We also request outside service providers to fax us the most recent Psychiatric Evaluation and Medication Sheet (if available) and any other documents or information regarding the individual’s mental health status, symptoms, and/or diagnosis.

Once we receive your call or online form, a pre-screening interview will be performed by phone and, if appropriate, a visit to the facility will be scheduled for the same or following day.

 

Transportation to and from the facility can be arranged for individuals interested in our Partial Care program. Once admitted into the Partial Care program, free daily transportation is available to most local areas.

Payment for Services

We accept New Jersey Medicaid, selected plans of Horizon BCBS, Amerihealth/Magellan, Out-Of-Network benefits from all other insurance carriers, and Self Pay.  We accept major credit cards. As a courtesy, we will check your benefits for you prior to your appointment; however, please remember that you are responsible to check your coverage benefits with your insurance carrier for more accurate information about your benefits.
