By accessing our Patient Portal, anyone requiring services will be able to register as a new patient. Our representative will contact you on the next business day for additional information and to schedule an appointment. If you are an existing patient, please call us to obtain your login for the Patient Portal. You will be able to register yourself or someone else (parent/authorized representative) on AFHS Patient Portal. Turn onto Clifton Avenue, then make a left turn onto Colfax Ave and then the following left turn on Orchard St and drive until you reach a gated parking lot.
From GSP
Take exit 154 to US-46 E, right away take the first exit on the right to Paulison Ave, make a left turn onto Paulison Ave until Clifton Ave and make a right. At Colfax Ave make a right turn and turn left at Orchard Street and keep going straight into the gated parking lot at the end of the street.
